Licensed Practical Nurse (LPN)
Overview:
The focus of this position is to encourage and provide complete client satisfaction while maintaining operational and clinical compliance for the branch office. This position completes all nursing functions as assigned by the RN and Branch Manager. This position reports directly to the RN.
Responsibilities:
Performs referral source-approved and private duty nurse visits under the supervision and direction of the RN;
Conduct initial visits to clients’ residences prior to the start of PC Service for the purpose of reviewing the Plan of Care with the client, worker and case manager under the supervision and direction of the RN;
Provide and document supervision of, training for, and evaluation of personal care workers according to requirements under the supervision and direction of the RN;
Provide supervision of the personal care worker at a minimum of every sixty (60) days for each client, documenting the visits in the individual client record;
Provide on-sight supervision of the personal care worker at a minimum of every six (6) months for each worker, documenting the visits in the individual client record and placing a copy in the workers' personnel file;
Assist Personal Care workers as necessary as they provide individual personal care service as outlined in the Plan of Care, documenting any supervision given in the individual client record, under the supervision and direction of the RN;
Assists with field staff supervision as directed by the Branch Manager;
Fills open shifts as directed by the Branch Manager;
Assists with in-service training and materials as directed by the Branch Manager;
Assists with field staff general training as directed by the Branch Manager;
Assists with investigations of worker and client issues in the field as directed by the Branch Manager;
Maintains nursing log;
Performs health assessments as needed;
Performs duties in accordance with all state regulations;
Opens client cases as directed by Branch Manager or RN;
Obtains Physician orders as needed and follows Physician orders when providing skilled care in the clients home;
Attends in-service and referral source meetings as directed;
Communicates with Physicians as needed;
Makes referrals as recommended; AND · Other duties as assigned
Minimum Qualifications:
Must be at least 21 years of age; AND
Licensed Practical Nurse in the State, on active status; AND
One (1) year experience Preferred as an LPN in public health, hospital or long term care nursing; AND
Demonstrated ability to assist in the evaluation of the Personal Care Worker in terms of his/her ability to carry out assigned duties and to relate to the client; AND
Demonstrated ability to assist in the coordination or provide orientation and in-service training to Personal Care workers on either an individual basis or in a group setting; AND
Demonstrated ability to provide appropriate follow-up regarding a client/caregiver and/or Case Managers dissatisfaction, complaints or grievances regarding the provision of PC Services; AND
Submit to a program for the testing, prevention, and control of tuberculosis annually; AND ·Possess a valid, picture identification.
Work Hours:
8:00am-5:00pm, but may vary as needed.
